Brief Answer:
it may be balanitis, antifungal drugs may be useful, circumcision best option

Detailed Answer:
Hello Sir,

Thanks for writing to us.

Patient with diabetes likely to suffer from infection of foreskin/penis.After age of 60 there is dryness in foreskin. With this dryness and being a patient of diabetes you are prone to some infection. In many cases chances of fungal infection are there. These factors lead to cracks inside of the foreskin.
Persistent cracks on foreskin depict chronic changes in that part of the skin. The cracks heal with fibrosis, making the foreskin tight to slide back on the glans when retracted specifically while erection. This leads to pain while having sex.Cracks reappear as tight foreskin stretches over the glans. And the vicious cycle continues.

For solution of this you should do the following things.

- keep good hygiene of penis and foreskin.

- do not wear too tight clothes/briefs

- apply moisturizer if there is dryness

- topical clotrimazole powder may be helpful if fungal infection is there.Apply
locally twice after cleaning.

If all above measures would not help , please consult doctor/dermatologist for examination.By chance medical approach do not wok successfully, circumcision is the best option.

In my opinion circumcision is the best option considering your age and diabetes. No need to worry for this minor surgery. It is a day care procedure and you can do all normal activities within 5-6 days without any problem.

Hope I have answered your query, I will be happy to help further.

Dear Doctor,
Thank you for the Reply.
I also observe small patches due to pigment loss on the foreskin. Is it also the result of diabetes or some thing else?

Kindly examine and reply.

Diabetes, fungal infection or malignancy can cause patches

Detailed Answer:
Hello,

Thanks for follow up.

Diabetes or fungal infection can cause patches on foreskin. Or it may be due to aging and pigment loss.

In rare case it may be indication of penis malignancy.
