Question: I think i have multiple melmona . Blood work. Iggkappa monoclonal protoen present . I have hpylori and diagnosed with Hyperparthroidism

You need further work up to search the cause of high calcium.

I have gone through your question. I can understand your concern.

Your calcium level remains more than 10. For your age it is definitely high. So you need to search the cause of high calcium.

Two most common cause of high calcium level are multiple myeloma and primary hyperparathyroidism. However combination of both disease togather is very rare condition.

Your PTH level remains at high normal range. So without clinical correlation its difficult to stemp the diagnosis as primary hyperparathyroidism. If you have osteoporosis, kidney stones and other symptoms then its due to hyperparathyroidism and you need surgery to remove your parathyroid glands.

Same time multiple myeloma can also leads to high calcium.

So you should go for protein electrophoresis, bone marrow biopsy and search for lytic lesion.

Please send me your reports of protein electrophoresis and bone marrow examination.

If it confirm the diagnosis of multiple myeloma then only you need treatment for that.

I read that if it was multiple melmoma that the pth level would be very low . In my case it is not is this true . Also my last lab the calcim went from 10.3 pth 60 to 9.9 pth 49.80 what does this mean ?

I have gone through your question. I can understand your concern.
If you have multiple myeloma then your pth should be low. It is true.

Generally calcium level is regulated by pth which is produced by parathyroid glands.

If your calcium level is low then your pth level is increased and it mobilise the calcium from bone into blood and your calcium level increased.


If your calcium level is high then your pth level deceased.

However in hyperparathyroidism pth level increased due to over production. That leads to high blood calcium level.


If you have multiple myeloma then your calcium level is increased by multiple myeloma that leads to suppression of parathyroid and pth level is decreased.

But you have high PTH level. So you may have both pathology togather or you have only hyperparathyroidism.

You should confirn that first. So please send me your reports so that I can help you further.

Your calcium and pth level decreased slightly. But that kind of fluctuations are common. It doesn't mean that its become normal.

In that way your level is normal. But at your age and in context of your previous report it is slightly high normal side.

But single level doesn't tell the true story. Your previous level were above 10. So it is significant.
