Based on your query, my opinion is as follows:

1. It appears to be bowel incontinence..
2. Stool impaction due to constipation or recent diarrhea episodes could have weakened the muscles of rectum. Diabetes can cause nerve damage and similarly affect sphincter muscle and cause bowel incontinence.
3. Fiber diet and adequate hydration to control constipation is necessary. Also pelvic muscles training, will be helpful in strengthening sphincter muscles.
