Hello, Dr Kakkar,

Uploaded 7 images (with some description in the image name itself).

I don't have itching or paining or burning sensations on that blistered areas as long as I don't touch it. Once I touch, it is mostly the sensitivity that is very high (as it close to the frenulum area). It is only about 25 days since I got circumcised.
I'm not using or taking any type of medications.

My urologist mentioned over an email (he didn't see me in person or photos), that this could due to natural tissue reaction & should slowly go away. He suggested me to use guaze sponge with some petroleum jelly to avoid such irritation due to rubbing against garments.

Question 1. :

I'd ike to get your opinion about the blisters (looks like water filled). Hopefully these benign like the new shoe blisters. And if these are due to irritated skin how do I calm it down and improve healing ?

Also reg. the report from urologist upon testing the foreskin (for BXO)

" Foreskin measuring 7 x 3 x 0.4 cm was sent to analysis and a White XXXXXXX discoloration was identified at the edge of the foreskin measuring 1 x 0.7 cm as BXO. It was not widespread, but involved a small patch on the outer edge of foreskin. Fortunately, the margins were all negative so no BXO tissue was left behind. I believe that removal of the disease segment is definitive . "


2. As per my urologist the BXO was localised on a tiny patch on the outer tip of the foreskin (which is now removed) and he doesn't visually see anything on the glans. So I'd like to get your opinion on the same. Also what should i look for in future & how do I prevent it ?

It does look like it is due to friction or rubbing. Prior to circumcision the skin of glans is covered with prepuce and is not keratinized but after circumcision it gradually gets keratinized due to exposure and friction and in the process may develop some irritation of this kind. I suggest you to just use a topical antibiotic e.g ether 2% mupirocon or 2% fusidic acid ointment, twice daily. This should subside naturally.

You may use an OTC topical antibacterial ointment e.g Neosporin ointment. The ointments which i had earlier mentioned i.e 2% mupirocin and 2% fusidic acid, respectively, are prescription medications.

Hello doctor,

When I try to urinate I do feel pressure build up for few secs and then it becomes fine. This happens say 2 times out of 5 times in a day when I pee.

Do you think the blisters and in turn the hardening of the irritated skin on the glans can cause this issue ? Btw it is about 30 days since my circumcision.

Hi.

After circumcision it is pretty normal to feel a little different while peeing for a few days. It is due to sensitivity of glans and it may take some time for you to get used to your penis without prepuce. I think that this should resolve with time.

Continuing with my earlier discussion ....

Earlier I uploaded pics from 13-Aug. After about 4 or 5 days I started to see blood blisters. There was also some mild skin peeling. Upload 4 new images for you to diagnose.
I uploaded new pics from 21-Aug (2 pics), 24-Aug (1 pic) & 25-Aug (1 pic).

I want to know if a fluid blister can turn into blood blister ? Or is this something else. Again as I said earlier I don't feel any other symptoms unless I touch those blisters (sensitive)

Also how long does this take to heal ?

Hi welcome back, Thank you for the pics.

I have reviewed the images and I am of the view that perhaps the blood blisters are part of LSeA affecting the underside of glans.

I referred back to the earlier images and I can make out white plaques (arranged in a reticular pattern) on the underside of glans extending on to the meatus as well and this is the exact site you have developed blood blisters on recently. So the blood blister is part of LSeA of glans.

In view of that, I suggest you use an ultrapotent topical steroid e.g Clobetasol propionate 0.05% cream, twice daily over the glans.

Dr. I'm now really worried. So you are saying this is BXO on the glans right ?

Can this be managed over time with this steroid ? I'd also make an appointment with both an urologist & dermatologist locally for a closer look.

BTW this could also be a blood vessel popped in the original fluid blister (from friction), right ?

Topical potent steroids are usually effective in LSeA apart from circumcision. There are no proven alternative treatments. I don't think it is just a popped blood vessel because I can also see some LSeA in the background.
