Question: sir,
My 7 year old son has a dermoid cyst on his head right on his midrib line.we noticed this since he was 3 months old. At age 3,the swelling was gone.but resumed back a year later.now it is constantly the same size.we did MRI scan at his 3 years.the report said there was no intracanial extension and the doctor said to remove whenever we find it suitable.please tell me a good doctor and a good hospital to consult. We are living in XXXXXXX XXXXXXX Is there any other ways to remove this dermoid cyst like in Ayurveda or something..without going through the surgery?
once removed, will the cyst appear again?

Also when my son was 28 days old my husband's leg by mistake( while sleeping on the bed) hit his head at the same part where the cyst was seen later. Does this have any connection with the cyst?
waiting for your reply.

Brief Answer:
Only surgery will help.

Detailed Answer:
Dear XXXX,

Thank you for posing your query on HCM. We aim to provide accurate information, but we are not yellow pages, you know. It is impossible for a doctor who can be from anywhere in the world to actually recommend A PARTICULAR doctor or hospital for any problem, and I am sure you would understand why this is also unethical.

The major question you seem to be asking is whether a dermoid cyst will go away WITHOUT surgery. That is clearly not possible here, as a dermoid cyst is an anatomical aberration of your son's head. It did not have any antecedent cause. The accidental injury he received from his dad when he was a month old has NOTHING to do with the cyst. As such, I cannot tell what Ayurveda can, or cannot, do, as I am not an Ayruvedic doctor. Suffice it to say, I would strongly urge you to take him for surgery as soon as possible.

If the cyst is removed, it will not recur.

The surgery for a cyst removal is quite commonplace, and barring any unforeseen emergencies, it is a completely safe surgical procedure.

With best wishes,

Dr. Taher

sir,
Please explain the meaning of..."as a dermoid cyst is an anatomical aberration of your son's head. It did not have any antecedent cause."
will there be any bad effects on my son, if the cyst is not removed? In future will
there be any harm to him, if the cyst remains like that itself.

And if i go for a surgerical procedure, will he be admitted in hospital for long?
How long will it take to heel? Please tell me the post surgery precautions to follow.

I also plan to do my son's circumcision, is it safe to do the both surgery together?
again please tell me the pre and post surgery precautions for circumcision as well. How long will it take to heel?

Will my son recover from both the surgery in 2 weeks time?what care should i keep in mind.?

Brief Answer:
Both surgeries are classified as "minor".

Detailed Answer:
First of all, thanks for reverting. I must tell you that I am not the right professional to discuss pre-operative, operative and post-operative details of the surgeries, as I am a medical pediatrician and not a pediatric surgeon. I advise you to raise the surgery-related questions with a pediatric surgeon for better apprisal.

Having said that, I wish to iterate that removal of the dermoid is a minor surgical procedure. However, surgeons often first do a CT scan or MRI of the head to determine if the dermoid cyst is only superficial (i.e., under the skin), or goes deeper. If it is superficial, removal of this is done under general anesthesia (the child is put to sleep and numbed). During such a surgery, there is a rare chance of surgical complications or anesthesia-related problems, but by and large, this is a safe operation and the child can be sent home within a day or two.

Circumcision is also a minor procedure. Doctors usually do it under local anesthesia (i.e., they inject medication in the private areas to numb the site, but if it is done with the other surgery, no further anesthesia would be needed and it can be done in the same sitting. Before doing any surgery, the child would undergo a screening of a medical examination followed by some blood and X-ray tests to make sure he does not have anemia (low Hemoglobin), any blood infection or any clotting disturbances. If all these are okay and his chest Xray is clear, the child is deemed fit for surgery.

Your child can be sent home within a few days even if both surgeries are combined. The exact operative and post-operative details would be discussed with you by the operating pediatric surgeon.

I hope this helps you.

Dr. Taher

P.S. By "no antecedent causes" I meant that no provoking or causative factors are involved in the formation of a dermoid cyst, which usually starts forming even as the baby is developing inside her mother's womb.
