Detailed Answer:
Hello XXXXX, Welcome to HCM,

I've gone through your query and understand your concern..

Please clarify the following so that the I can guide you more specifically :

Is this concerning you or are your asking on behalf of any other individual(if asking for any one else then please briefly describe the details of the concerned person i.e. age/sex, current health status, past medical history, on going medications etc.)

If this is concerning you then please let me know if there's any excessive sweating, which part of the chest is affected, is there any sudden heaviness over the chest also, is the pain increasing during breathing ?

Any past/present medical/surgical history ?

Any history of trauma ?

Regardless of the facts, since you've mentioned there's no doctor or care giver available right now, i'd strongly suggest you to call for help..

There'll be need to go to your nearest ER/Hospital for an immediate medical attention, for which please seek and find out the helpline number (every hospital has one), you can easily find it out over the internet and call them up at the earliest..

Please, don't panic and let me know these details so that I can guide you more specifically..

Brief Answer:
can be several reasons, including a lung parenchymal injury..

Detailed Answer:
Dear XXXXX, Welcome back,

Thanks for following up with the details,

It can be due to several reasons including a secondary neuro-muscular spasm to even lung parenchymal injury, the most concerning one is a Pneumothorax (specially considering your age and secondary to the act of Smoking hookah..)..

Yes, it's an Emergency, please seek for help, don't try to lie down, remain seated in up-right position, try to take small breaths..

Don't try to take deep breaths, Inhale slowly, exhale slowly, and please don't delay any more and visit your nearest ER/Hospital at the earliest..

There's a prompt examination needed followed by most importantly a Chest X-ray to rule out any underlying lung injury at this point..management will be as per the findings..

Please don't delay any more and call for help dear, don't panic, but this needs to be ruled out at the earliest..

Let me know how it you stay later on..

Take Care
Kind Regards
