Question: Hi, my daughter is 22 years old, she got severe cold and coughing 2 months back along with pinching pain in middle of chest. Got the treatment from family doctor suggested Maclov antibiotics with some cough syrup and acidity tablets. After 15 days Coughing continued and small traces of blood in mucus observed in the morning while forcefully taking it out during tongue cleaning. Hence went for cheast X-ray and found that there is no congestion. Blood report with thyroid test came normal. Doctor suggested to go for course of Antflu 75 mg for 6 days which completed and coughing reduced. However the middle cheast pain complaint continued, for which doctor suggested anti acid tablets and syrup. However the punching in Cheast vertically and right side horizontally. It was not continuously through out the day but when it pains it give sharp pinching effect. We decided to go for CT scan thorax and CT abdomen and povel. There are few fibre strand and I'll defined nodular opacities in right middle lobe, we consulted pulmonologist and he saw CT films and found that it's normal and chest pain is due to acidity or infection and bronchitis. He prescribed medicine I. e. Odiment LC tablet, Tablet D capsule in the morning at empty stomach, Vit. C tablet. Syrup tossed and Maclov antibiotic 250 mg for 5 days. He informed that the pain will go with these medicines. Please suggest.

I can understand your concern.
First of all no need to worry for lung infection or tuberculosis.
For bronchitis, PFT (Pulmonary Function Test) is must.
But I want think bronchitis is less likely because she is not having expectoration and breathing difficulty.
But better to get done PFT first.
Another possibility is costochondritis (inflammation of ribs and rib cartilage).
So along with vitamin D and C, also apply warm water pad on affected areas.
Tell her to avoid heavyweight lifting and strenuous exercise,avoid sudden jerky movements of chest and sudden bending.
If she used to sit a lot then sitting with proper back support is needed.
She can also apply volini gel or Enzoflam gel on affected areas.
She can also wear rib belt to prevent mobilization of affected parts.
She should take pregabalin and vitamin B12 combination for nerve pain.
In my opinion, she will mostly improve with all these in 10-15 days.

Thanks for your reply. As I explained that she has no coughing, no fever, no weigh loss, no vomiting, no nausea feeling. She eat well & digestion is also good.

Also, as mentioned, after we consulted Pulmonologist 3 days back, she started taking medicines i.e. Odiment LC (Anti-allergic medicine), Maclar 250 (Antibiotic), Vitamin C & Rablet. She also started applying Hot pad & Volini Gel on pain areas as you advised from day before yesterday. She also started eating one Banana.

Yesterday morning she felt good & did not complain about pain. She had breakfast in the morning. However, when she had hot chicken soup & Chicken pieces in the meal, the pain again started & continued till late evening.

I would like to inform you that the pain varies, some time it is in middle of chest & sometime it is in upper right or left side of chest near shoulder.

She applied hot pad & volini gel yesterday night. Today morning, she had little pain but less compare to yesterday. Also, when she drink water, she felt good.

Today evening, when she had dinner, she complained about little pain while swallowing food & then pain occurred in middle of the chest.

She has completed three days dose of medicines prescribed by Pulmonologist but there is no relieve from pain. There are no other complains. Please let us know the what are the reasons & remedies. Is it possibility of Gestric ulcer or Ulcer in mid of food pipe? OR Is there Lungs infection? OR is it costochondritis as you informed. I informed you that CT scan is normal.

Hello, Thanks for the quick reply. The answer of your questions are as follows.

1. Is this pain associated with nausea or burping?

No, Pain is not associated with nausea or burping. Infact there is no nausea feeling. The pain is keep coming & going. However, she gets temporary relief from the pain when she apply hot pad, drink water, take rest (sleep).

2. Is this pain recurrent? I mean always associated with spicy food.

Yesterday, she had hot chicken soup & chicken pieces in restaurant but the same were not spicy. Then yesterday evening pain was there & she had home made Khichadi but she did not feel any pain while swallowing. Today, she had lunch at home (Roti, Mung dal & green vegetables) which were not spicy & she did not feel pain while eating. In the evening, she had again pan fried roti with mixed vegetables (Potato + Brinzol + Methi) but she felt pain while swallowing.

3. Is this pain subside with antacid (pantoprazole or rabeprazole)?

She takes Rablet (contains Rabeprazole) while empty stomach in the morning. However, when she get up in the morning, she does not feel much pain but today she forgot to take Rablet hence it is difficult to say whether pain subside with it. She takes Rablet in the morning & then Milk with Banana with light breakfast then she takes anti-allergic tablet & Maclar 250. However, she keep saying that the pain is little but reduced compare to 3 days back.

In my opinion, you should definitely give her Rabeprazole on empty stomach before dinner.
This will help in gastritis related pain after dinner.
Also, make sure that she eats less hot and spicy food for dinner.

If by doing these things she improves then a possibility of gastritis related pain is more likely.
