Brief Answer:
Lichenification

Detailed Answer:
Hi,

You seem to have had eczema around the anus. The reason for eczema could be many, be it parasitic infection, atopy or fungal. You had scratched it because of intense itch that it gives you. So over a period of time the skin has become thick leathery to touch and brown in color. This is called Lichen chronicus. Since I did not inspect it, I could guess these chain of events.

The treatment is simple, high potency steroid which needs to be applied and area should be covered with gauze for better delivery of the drug and also prevents damage caused by scratching. Some recent immunomodulation drugs like Tacrolimus can be used.

So the treatment needs a visual inspection of a Dermatologist who can prescribe you the high potency steroids like Doxepin to help stop scratching.

Hope this is clear and understandable. I would love to assist you further.
