Could Nexito Intake Cause High Blood Pressure?

Question: I am 47 years old Male having 80 Kgs.I was not having any health issues by god's grace.Lipid profile was normal.My blood pressure used to be 110/80.Due to heavy work at office and to control anxiety,My Doctor (MD)suggested me Tablet i.e Nexito plus.I used it for 20 days.While using it, I developed blood pressure up to140/90. Again I went to same Doctor.He gave me Metpure-XL25Mg and I am using it for last 2 weeks. I am not feeling much better.Bp is now 135/85 or 130/90 some time s. If I go office,Bp is still going up. I strongly believe that everything is because of Nexito plus which has disturbed my thyroid and because of that BP is shooting up.Pl confirm is it correct.What are the tests to be done to ascertain BP apart from lipid profile.Should I continue taking Metpure XL25 or should I increse dosage or should I change Tablets or should I consult Cardiologist.Pl suggest

Hi,

I understand your concern. Nexito plus was prescribed to control anxiety. This drug as you might know is a combination of escitalopram (SSRI) and clonazepam. And you noticed raise in blood pressure levels after starting the medication. My comments in this regards are as follows:

1. Nexito plus commonly doesn't affect thyroid levels. But in rarer circumstances (there are some case reports) where it may lower thyroid hormone level. However hypothyroidism isn't known to cause hypertension. In a nutshell, your belief that Nexito plus could have affected thyroid hormones and thereby increased blood pressure may not be absolutely true. However you can still get thyroid functions tested to check status of thyroid hormones.

2. That being said, Nexito plus can alter serotonin levels. This can cause BP changes. However in most cases the effects wean off gradually unless altered again by drug changes / interactions. So you can keep a watch on BP changes. BP medications can be altered accordingly in future.

3. Since BP readings are under limits now, I would suggest to continue both medications. Follow up with your doctor on regular basis.

Thanks for your information.
(1)As you said, Nexito plus can alter serotonin levels which can cause BP changes. Is there any Medicine to counter (undo )this effect as I already used this Nexito for quite some time (20 days) or any medical test to know the level that effect
(2) I started using Metpure XL25MG for last 10 days,When will it will show its effect ? My BP is still 135/85 and when I am in office ,It will be 140/90.Or else Should I change its Metpure XL25 MG to some other.
(3) My boss is good for me but he always shouts on others.Whenever My boss calls me,my BP levels shoot up though he does not tell me any thing.Can we control this by taking medication ?
(4) As I am still staring stage of using Alopathy medicine,Can I start using Homeopathic Medicine apart from Alopathic medicine.

Hi,

1. Serotonin changes are for your benefit. It helps to control anxiety. Yes, the BP changes are a side effect. And the side effects usually settle down over a period of time. If the BP continues to be raised, perhaps it may not be related to Nexito plus.

2. BP of 135/85mmHg is in the upper limit of normal. So you need not be overly concerned. Worries and stress are going to further affect BP. If it persists to be high, I would increase dose of Metpure XL than change it.

3. The situational anxiety you referred to should get under control with medications prescribed. You can also try transcedental mediation and breathing exercise if necessary to control anxiety. Exercise early morning, yoga and good sleep will also contribute to the treatment of anxiety.

4. I do not have any knowledge on Homeopathic drugs. There are not robust evidences that they help. However if you wish to start them, you can discuss with homeopathy specialist.

Thank you much for service. I am extremely satisfied with your timely replies.

Further, I am supposed to Take Metpure -XL25 soon after Break fast.

(1)If I take Metpure -XL25 soon after break fast ,I will be so much dull and do not feel like anything till afternoon.

If I take half tablet i.e 12.5 MG in the morning and balance half, after lunch,it will be comfortable.
Is it ok ?

(2) I understand ,long usage of Metpure will give increase chances of becoming Diabetic ?
Pl confirm.

(3) Is there any expensive medicine which does not significant side effects to control BP.

Hi,

Choice of BP medicines is individualized to patients need. In your case presence of anxiety prompt the use of beta blockers (metpure XL). As long as you are going to take the same halves as morning doses before and after food then it's ok. You shouldn't be taking first half of one tablet and another half of different metpure XL tablet.

There are reports that beta blockers elevates blood sugar but I'm unaware of robust studies that confirm the same. I have used it on many patients without significant side effects.

I would switch to a calcium channel blocker like amlodipine over the due course as anxieties are under control. You can discuss this medicine during follow up visit.

My doctor has adviced me one month back to take (1) Metpure XL-25 Mg in the morning and (2) Rivotril 0.5 Mg in the Night.These tablets prescribed for Pre Hypertension .
I am taking only one Tablet i.e Metpure XL-25Mg in the morning. I am not taking Rivotril 0.5 Mg.
I am feeling weak and drowsiness. Hence I checked my sugar levels which are coming down.My doctor is out of station
Pl clarify the following
(1)Is it because of not taking Rivotril 0.5 Mg.
or
(2) Is it a side effect of Metpure XL-25Mg
(3) Should I change tablets to over come this problem
(4) I am very much worried, Whether I became diabetic or what

My all reports (CBP,Lipid profile and urine examination) are normal. I wanted to upload them ,but there is no provision here.
Fasting sugar was 80 and Post lunch was 93.This was done 15 days ago.

I got it checked again when I was feeling weak and sleepy,my post lunch sugar was 79.This was done on yesterday.
Threedays ago, I got giddiness and aftersome time I became normal.It has developed after I started taking Metpure-XL25MG. My earlier reading were 90 (Fasting) and 120(Post lunch)

Remote possibility of being related to metpure XL

Hi,

A few studies recorded increase insulin sensitivity and plasma insulin levels with use of metopralol (Metpure XL). This leads to hypoglycemia. However the studies were done in patient with diabetes and hypertension.

You are not known to have diabetes. Therefore, I am not sure if this was the cause of low blood sugar levels. In this context, it would be worthwhile to screen for diabetes. Please get your fasting blood sugar as well as post prandial blood sugar (post breakfast) and Hba1c tested from a reputed (NABL certified) laboratory.

In the interim you can report back to the doctor and discuss about switching to other antihypertensive drug. That being said, Metpure XL is a very good choice since you have history of anxiety.

Accordingly, blood sugar changes are not related to rivotril or the fact that you are not using it.

Hi,

It seems the information you provided about post lunch sugar being 93, 15 days ago was not true. 93mg/dl was your sugar value post breakfast. It is good to compare sugar values during the same session to see for blood sugar behavior and changes.

You can compare post lunch blood sugar with a glucometer if you have one on two different days with similar lunch meal with and without Metpure XL. It will give you an idea if the drug is indeed lowering the blood sugar.

How is your sleeping habit? Do you sleep sufficient hours at night?
Have you started Nexito plus that was prescribed to you earlier?
