Severe pain in the lower right quadrant, of sudden onset, associate to fever and loss of appetite are characteristic of acute appendicitis. This is a surgical emergency and calls for immediate surgical intervention. Irritated hernia, if in the same area presents with similar symptoms. However, there is often a history of hernia before it becomes irritated.

After a heavy workout at the gym yesterday around 5:00, continuous pain started under rib cage (right side) down to the top of my leg. Pain never let up all night
and this morning. I have an abdominal hernia on right side that slips out about once a week. I push it back in and hold for about a minute. Could it have ruptured or could it just be irritated? Is ice good for now?

With all this pain and the surrounding circumstances, I will suggest that you get it evaluated. It might require that we carry a surgery to get this rectified. Relieving pain alone at this time might not be sufficient.

Pain went almost away on the third day. I am thinking it was my workout at the gym. I must have pulled something, but I will schedule an appointment with my
doctor anyway.
