What Do Swelling And Pinching Sensations Under The Ribs Indicate While Defecating?

Question: Hello,
I have seen six different doctors for a persistent and irritating problem and don't feel that I've been given a proper diagnosis. Since having my second child (both csection) I have a swelling in my intestine in my lower right quadrant as my body is preparing to Deficate. I feel a great pressure, much like the feeling of a baby pressing outward while in utero and can see a bulge. I can push it back in, but then I feel the pressure and almost a pinching feeling just beneath my rib cage towards my side. This happens almost daily or every other day. It is important that I mention, my intestines are not coming through the muscle. It is almost as if they swell up and get tangled on the inside, or caught on something towards the midline of my abdomen. One doctor told me this was gas, so I stayed away from gassy foods, took gas relief pills, and incorporated more fiber into my diet, and saw no changes. I saw another doctor and they told me it was from stress. So I left my job that was stressful, took a vacation and still saw no relief. Another doctor told me it was a hernia, but that when I had my tummy tuck it would be resolved because the intestine was popping up through my midline, my plastic surgeon sewed it up tight and now the bulge is smaller but I still feel the swelling/pressure and can see a small lump. I saw another doctor and she said that it was simply because I had poor abdominal muscles, and suggested that I wear a binder to keep it from happening until I can strengthen my muscles... when wearing a binder it still swells up and I can feel the pressure. I obviously just can't see it because there is a binder. I also worked out quit a lot before I had my surgery a few years ago. I am 5'7 148 lbs. I'm not sure if according to her suggestion I just have to be a body builder to have this resolved. I am becoming increasingly frustrated at these diagnosis' because so far It seems my medical advice is that I just need to be a body builder on permanent stress free vacation while eating fart free foods. I am 28 years old. I NEVER had issues like these until after my second csection. I feel as though they didn't put my guts back in correctly and they are tangled or something. If anyone has experience with this issue or suggestions as to what I should consult my doctor with I would greatly appreciate it. I can alsnprovide a video of the bulge rolling back into my abdomen.
