Question: I am diabetic. I took a dose (50 units) of Humalog fast acting insulin instead of the slow acting Toujeo. What should I expect the reaction will be. It's been about 45 minutes. No reaction as of yet.

Brief Answer:
Emergency

Detailed Answer:
This is a potential medical emergency. You are likely to experience a significant drop in your blood glucose levels.
You will now need to be extremely vigilant for the next 4 to 6 hours. I say 4-6 hours because typically the effect of humalog insulin lasts for this time frame in the human body. Of course it tends to vary from person to person, and with various medical conditions, for example in those with kidney problems, it tends to last even longer.

At any rate, now you must contact your healthcare provider and ask for instructions and guidance on further management.

When my patients make such a mistake, I advise them to check blood glucoses at home every 15 to 30 minutes for 6 hours to ensure they are not going low below 70 -100 mg/dl. I also ask them to eat a large meal, and keep consuming milk/regular soda/ orange juice / sugar tablets or gels each time blood glucose drops below 70. The goal is to keep the blood glucoses above 100 consistently till the effect of humalog wears off. It is important to not overdo it either , meaning do not exceed blood glucoses above 250 mg/dl by over-eating or over compensating
