Posted on Tue, 27 Jun 2017
Question: I am on Trintellix 20mg q am, 100 mg gabapentin tid and 7.5mg mirtazipine at HS for sleep. I also have 0.5 of Ativan prn and see a therapist regularly. I want to get off mirtazipine d/t difficulty getting up in the am, grogginess & muscle aches. I've been on it for approx. 6 months. I've tried before and was able to sleep but became weepy & depressed; I believe this is from discontinuation. How can I get off the mirtazipine successfully? I have read online about other patients experiences and am scared to try again but can't continue feeling like this.

Brief Answer:
It can be done in gradual manner

Detailed Answer:
Hello,
Thanks for using Healthcaremagic.

I read your query and understand your concerns.

First of all I like to inform you that there exists a plan to discontinue mirtazapine and if it is followed properly there is almost nil chance to have discontinuation symptoms. I also need to acknowldge that is better to discontinue in gradual manner to have least possibility of withdrawal symptoms.

You can do in following manner
Day One
Three fourth tablet of 7.5 mg mirtazapine

Day 11
Half tablet of 7.5 mg mirtazapine

Day 21
One quarter tablet of 7.5 mg mirtazapine

Day 31 stop

I must also inform you that the tablet of mirtazapine is unscored and you may get some difficulty to break it in pieces in precise manner. However breaking it in approximate pieces must be enough for you as for most individuals.

I hope this helps you.
If you have any further query, I would be glad to help you.
If not, you may close the discussion and if possible you may rate the answer for my future patients.
