Suggest Ways To Regulate Hydrocortisone To Even Out Highs And Lows

Question: I have been diagnosed with Addisons disease and am depressed because I run a business and cannot do my job. I had a seizure while having my gall blatter removed and the docs. think somehow it interrupted the transfer of Acth to my adrenal glands. I am 61 typeA and now alwaystired fatigued etc. I can't regulate the hydrocortisone. Too much and I get road rage too little and I sleep all day . Tried 5 mg. prednisone and dexamethasone. What can I do to even out the high and lows through out the day and sleep at night. Very depressing having it diagnosed at my age. Used to be active pilot, scuba instructor , teacher , now can't seem to get a handle on the right amount or type of medication. Can you recommend a really good endocrinology doc. or advice.

Hello welcome to HCM.
I have read your question and understand your concerns.
It's bad that you have been gone through such a phase, but after proper replacement of hormones you can live a normal life.

My advice to you will be

1. To start prednisolone 5mg in the morning around 7am, half tablet of 5 mg(2.5mg) in the afternoon around 1pm. It will usually cover steroid requirements in my patients. Discuss about this schedule with your doctor. As such proper dosage should be adviced by your doctor only after your proper examination and seeing all reports.
You also might need fludrocortisone in replacement of deficient aldosterone in your body, particularly in summer, ask your doctor about that.

2. During excessive stress, disease, surgery or in infection your steroid requirements must be increased sometimes 2 to 3 times , for proper dosage at that time you have to consult your doctor, preferably endocrinologist.

3. Do your ACTH levels, serum electrolytes after 1 month. Also check your blood pressure. If the reports are within normal limits then continue the same dosage.
