Suggest Treatment For Vomiting, Indigestion And Diarrhea

Question: Sir,
My daughter aged about 11+ years (weight 48 kgs) has been suffering from chronic stomach problem since April 2015. During these 2 months she had 3/4 episodes of diarrohea coupled with vomiting and indigestion. Initially I gave her ORS and Loperamide and Domperidone for vomiting . With these medicines she is cured and attend her normal school. But again after 2/3 weeks she had similar symptoms i continued with same treatment , it worked. Since yesterday again she had similar problem. She vomitted undigested food yesterday morning before school. Since she had exam, so I gave her domperidone 1 tab and she went to school. But after coming from school , she had 5/6 loose motion and ultimately I gave her Loperamide 4 mg at bed time. She slept well but this morning again she went to toilet for 2/3 times. Vomiting tendency is not there but she is reluctant to take ORS. I have just given her Loperamide 2 mg. Kindly give your necessary advise so that she get relief from this chronic problem.

Hi

I have gone through your question and understand your concerns. It seems that your child has been suffering from gut infection most likely giardiasis. The treatment you have been giving is for symptomatic relief but it doesn't help in treating the condition completely. I would recommend you to:
1. Get her stools examined- routine and microscopic examination, especially for ova and cyst.
2. Start her on Tablet Metronidazole thrice daily for next 7 days. This will clear up the infection. However this is a prescription medicine. You should take this after discussing with your local doctor.
3. Give her Tablet Zinc Sulfate ( with 20 mg elemental zinc) once daily for next 1 week. Zinc helps in healing of diseased gut mucosa.
4. Vomiting episodes seems to be due to associated gastritis. She will benefit by using T. Lansoprazole once daily before breakfast. Discuss with her treating doctor.

Along with this, give her regular non spicy and healthy diet with plenty of fluids.

Thank you, Doctor, for your quick reply. I just have one question regarding the suggestions you have given. How many mg of Metronidazole should she be provided with? There are 200mg tablets and 400mg tablets available in the market.
