Brief Answer:
vaginal candidiasis.

Detailed Answer:
Hello,
Thanks for the query to hcm,
This is evident by the sigs and symptoms of vaginal itching , accompanied with swelling and curdy white vaginal discharge that she has contracted vaginal candidiasis again after her initial treatment , where i believe that only she was treated and you were not offered any treatment.
This is probably the cause of recurrence now. The fact that you too have developed rashes on the tip of penis indicates that possibly you were harbouring the fungus infection infection, though asymptomatic then , but now both seem to have been affected. Hence it is always a practice that in cases of vaginal candidiasis with vaginitis ( possibly due to accompanied bacterial infection as well), both the partners should be treated simultaneously as this prevents recurrence to a great extent,

The medicines taken the last time should be repeated again for a week by your wife, but this time i would suggest you as well to take TAB FLUCONAZOLE 300mg stat once a week for 2 week and apply CLODERM ointment over the tip of penis thrice a day for 5 days.
Avoid intercourse for a week so cross-infection is prevented. I am sure both of you would be healthy sooner.
Regards
