Suggest Treatment For Uterine Fibriods And Heavy Periods

default
Posted on Fri, 24 Jun 2016
Question: I had a myomectomy this past february for uterine fibriods and heavy periods. Two fibriods were removed but the doctor missed a fibriods and heavy periods have not resolved after the surgery so far.

I got the results of my edometrial biopsy and it showed negative for anything such as endometreosis, hyerdysplsia or cancer. And I had a saline ultrasound as well that showed there was a 3.5 fibriod in the uterine wall that was somehow missed on the previous ultrasound and during the myomectomy. I have started take depo to control heavy bleeding. I'm very upset that the doctor missed this fibriod. I feel she should be sued for not doing further advance screening before the surgery, because now I have no improvement of symptoms after major surgery and now have to find a way to control the bleeding long term. I;m not sure depo will be a good long term solution. I heard you can only take it for a couple of years.

Brief Answer:
Don't panic, LNG IUD is a effective and rational option

Detailed Answer:
Hi, Dr Purushottam Neurgaonkar here. I welcome you to HCM VIRTUAL CLINIC. I have gone through your question, and I think I have understood your concern. I will suggest you the best possible treatment options.

1 ) First of all do not panic.

2 ) It can happen in the medical field that there can be limitations of diagnosis or treatment. One should understand that medical science is the most incomplete and ever changing branch of the science.

3 ) Please do not consider that I am trying to pacify you. But on an average no prudent doctor ever treats the patient with an intention to harm the patient.

4 ) Many times pre operative small sized myoma / fibroid which were undetectable before grow up later on. Speed of growth is also unpredictable. This Might Have Been the most possible reason of your third fibroid.

5 ) As on now, I will suggest you to use Levo Norgestrel containing IUD. This device can be fitt inside the uterus by gynecologist. It has shown to be very useful and effective. It has various advantages over depo injections. You can consider this option.

I wish you a healthy and fast recovery.

I hope this answer helps you.
Thanks

Hi Dr,

I feel like she could have found the fibroid if she had used more than just a regular ultrasound before performing surgery. This could have potentially saved me a lot of trouble and further complications. Shouldn't a doctor do an MRI or SIS or hysterscopy, more than ultrasound before doing any operation for fibriods being that they are so easy to miss?

Also, can you please explain to me the various advantages the IUD has over depo for controlling heavy bleeding due to fibriod and or hormonal imbalance?

Is it possible the doctor failed to remove what was already known about, and is it possible for a small undetectable fibroid during surgery to grow to this large of a fibriod in less than 2 moths post operation?

Thanks

Brief Answer:
LNG IUD reduces blood loss. very less drug dosage

Detailed Answer:
Hi there, I thank you for the appreciation and reverting back.

I can understand your feelings, and the trouble that you are facing . I agree with your suggestions. But let's try to find out best possible solution in the given situation.

Let me explain the advantages of the IUD. It contains Levo Norgestrel 52 mg only. It is released by a special delivery system, so drug is released over the period of 5 years. This significantly reduces the drug dose. Depo is 100 mg injection.

Secondly, IUD Helps to reduce blood loss by thinning out the endometrial lining and reducing the size of the fibroid.

I hope this answer helps you.
Thanks
