Suggest Treatment For Upper Respiratory Tract Infection

default
Posted on Mon, 4 May 2015
Question: I've been tested last month with spirometry (completly normale) and methacholine (negative).
Lungs clear just a little hoarsness with sore throath. I just got this throath cough with brown mucus.
I'm smoking about ten hand-made cigs at day.

The nurse and the doctor that tests my lungs says that there's nothing wrong and that a negative methacholine means "no asthma at all", but my family PhD put me on Spiriva (25/125). It seems that my throath symptoms are just getting worse.

Brief Answer:
You might be worsen due to poor inhaler technique.

Detailed Answer:
Thanks for your question on HCM.
I can understand your situation and problem.
Since your lung function test was normal, no need for spiriva in your case.
Your throat symptoms are mostly due to upper respiratory tract infection (URTI).
Smokers are at increased risk of URTI due to constant irritation from smoke.
And spiriva inhaler if not taken correctly, can worsen URTI.
Drug particles released from inhaler can stick to posterior pharyngeal wall, if technique is not good or you are not doing gargles after inhaler use.
These particles can irritate pharynx and cause inflammation. So, this is the probable reason for your worsening after inhaler use.
Please let me know
1. Do you have spiriva meter dose inhaler (MDI) or dry powder inhaler (DPI)?
2. Do you follow proper technique for use?
3. Do you gargle after use?
Please reply me answers of above asked questions, so that I can guide you better.
I will be happy to help you further.
Wish you good health.
And quit smoking as soon as possible.
Thanks.

I use the meter inhaler, i follow all the instruction teached by my PhD and gargle after any use.

But the fact is, i really need to cure an inexistent "asthma"? For what i've understand the methacholine challenge is the golden standard for ruling out the disease, am i right?

Brief Answer:
Yes, you are right about methacholine test.

Detailed Answer:
Thanks for your follow up question on HCM.
If methacholine challenge test is negative then no need to worry for asthma and no need for inhaler.
The only thing you must do is quit smoking as soon as possible.
It is not a good habit.
So better to leave it as early as possible.
Hope I have solved your query.
I will be happy to help you further.
Wish you good health.
Thanks.
