Question: Hello Dr. XXXXXXX - As per your advise, I am taking the following medicines for treatment of my headache :-

1. Betacap TR 40 (in the morning)
2. Tryptomer 25 mg (at night)

I wish to inform you that I am also suffering from the continuous air-cutting sound (perhaps it is called tinnitus) in my left ear. This problem started six years back only when I got the surgery done for reconstruction of my left eardrum. Of late, I have noticed that the sound volume has increased causing a lot of disturbance which invariably leads to headache. Earlier I forgot to share this information with you. Please advise if this could also be a reason for my continuous headache. If yes, what should I do to get rid of this as it is very disturbing particularly at night. Please help. Regards...XXXX

Welcome to this forum.
Unilateral tinnitus with a history of tympanic membrane repair means involvement of middle and inner ear.
What was the symptoms leading to ear perforation?
Any ear discharge?
Any hearing loss?
You need to get BERA and Audiometry done.
Brain MRI may be required to look for any abscess leading to headache.
As you have responded to the migraine medications, possibility of abscess is less likely.
Upload any investigations or discharge summary of your ENT problem.
You can take vertin 16 mg thrice daily for response.

You have asked the following questions:

REPLY : In December 2009, I had acute congestion and throat infection. I developed pain in the left ear. The ENT surgeon pierced my left eardrum to relieve the pressure. After perforation, I developed acute infection and massive discharge started from my left ear. The discharge continued for almost 15 days. I was told that perforation will close automatically, but due to the acute infection post perforation, the hole in the eardrum did not close. Ever since the perforation was done in December 2009, the air-cutting/buzzing sound started in my left ear. I went for another surgery in April 2010 (tympanoplasty) for re-making of the eardrum. Discharge summary issued by Sir XXXXXXX XXXXXXX Hospital is attached. I was told that after tympanoplasty, the air-cutting/buzzing sound in the left ear will disappear. But even after tympanoplasty the sound in the left ear continued. Hearing is normal in the left ear.

Now I have two issues bothering me....1) Headache and 2) Buzzing sound in the left ear. For Headache my earlier neurologist Dr. Bansal got the Brain MRI and EEG done. Both the test reports were normal.

Headache is slightly under control with Betacap TR 40 and Tryptomer 25 mg. Please advise medicine to get rid of buzzing sound in left ear and headache which invariably bothers me. Please let me know if I need to get some further tests.

The tinnitus may be due to involvement of inner ear.
You can get BERA testing done.
You can try vertin 16 mg thrice a day.
Get back again after 2 weeks.
