Posted on Tue, 13 Jan 2015
Question: My brother is 42 years old, and currently lives in Saudi Arabia. He suffers from a chronic Asthma, and he takes several medications such as Singulair, Clarinase in addition to the steroids. His doctor recently discovered that he has stomach inflammation and prescribed Pantoprazole for temporary usage. It seems his medical condition is not getting anywhere better for several years now, so I would like to get a second opinion on his condition specifically in regards to the best medications to take, lifestyle, home or alternative remedies and if he needs a specific medical examination that you would recommend.
Thank you

Brief Answer:
start inhaled treatment.

Detailed Answer:
Hello dear, thanks for your question on HCM.
In my opinion, you should first consult pulmonologist and get done chest x ray and PFT ( PULMONARY FUNCTION TEST ).

Worsening of asthma can be due to infection. So chest x ray is needed.

And PFT is must to know the severity of asthma. And treatment is based on severity only. So get done chest x ray and PFT.

Singulair is plain montelukast which is anti allergic. Clarinase is loratidine which is antihistamine drug. Both are add on drugs for asthma, not main drug.

Main drug in asthma is inhaled bronchodilators and inhaled corticosteroids(ICS ).

Long Acting Beta 2 Agonist ( LABA ) and ICS when given in combination, gives excellent control. As action lasts for 10-12 hours. So only twice daily dosing is needed.

Oral steroids are not used routinely as they are very toxic. And his side effect of gastric information is due to steroid only. So better to avoid oral steroids and start inhaler therapy containing LABA and ICS.

Short course of antibiotic may also needed if infection is present.

Asthma is triggered by allergen. So identification of allergen and if possible complete avoidance is needed. By this lifestyle modification , asthma control can be achieved.

Start chest physiotherapy and deep breathing exercise. This will help him in strengthening lung functions.

The most common cause for asthma attack is infection. So better to take adult pulmonary vaccine like pneumococcal and influenza vaccines for prevention of future lung infections.

So better to consult pulmonologist and discuss all these. Stop oral steroids and start inhaler treatment.

Hope I have solved your query.
I will be happy to help you further.
Wishing good health to your brother.
Thanks.
