Question: Hi
I have a urinary tract infection and have taken two sets of antiobiotics and it still has not gone..one lot is macrodantin and also I have taken a very strong antiobiotic augmentin..but it has still not cleared..perhaps you can look at some of the other information on this site and see if you could see if there is anything triggering this.i also have very sore breast.s.. I am fifty years of age

Hello,

I suppose it's a renal infection (that's what you've mentioned in other questions). In cases like this neither nitrofurantoin nor augmentin are the best choices. I usually prefer ciprofloxacin for community acquired infections, which has a great efficacy in the urinary tract.

An ultrasound scan of the urinary tract may reveal tumors, renal stones, abscesses etc. It's very important to do it because it may change the treatment plan.

I can't find any link to the neck MRI.

So in conclusion, the patient with an upper urinary tract infection has to be stabilized and treated as soon as possible. A potent antibiotic and relevant lab tests would be needed to fully assess the patient's status.

You've mentioned that the fistula originates from the intestine and ends up in the skin. If it won't involve the urinary tract then there is nothing to link the urinary tract infection with the fistula.

Common symptoms of upper urinary tract infection include high fever with rigor and flank pain. Sometimes only fever is the presenting symptom. If renal stones or other causes of obstruction are involved then urinary tract symptoms like frequency and urgency can be reported.

The ultrasound scan should investigate the urinary tract starting (kidneys-ureters-bladder). It is important to identify two working kidneys, no obstruction, no abscesses, etc. Normally in an uncomplicated infection, no abnormal findings should be reported.

Yes, the ultrasound scan is important because depending on the findings, a different treatment approach might have to be adopted. For example an abscess would need drainage. An uncomplicated infection would only need antibiotics.
