Brief Answer:
It is likely to be tendinitis.

Detailed Answer:
Hi,
Thanks for your query.

I'm Dr. Sudhir Kumar,

After going through your query I concluded that you may be probably having tendinitis as you are having very extreme sore spot on the upper portion of the shoulder.

I want to you few more questions :-
1.Whether you overexerted it by strenuous work when it started several weeks ago?
2.Where it is located anterior, posterior or exactly on top ?

Till then its treatment supposing it tendinitis is rest to joint.Don't involve in activities causing pain. Analgesic such as ibuprofen is given for pain relief three times a day.Hot fomentation is done at least twice in a day.Vitamin B and C is helpful in recovering. You can discuss with your treating Doctor about it.

Hope I have answered your query.
If you have any further questions I will be happy to help.

Wishing you good health.
Take care.
