Brief Answer:
Seems to be suffering from DENTAL ABSCESS.

Detailed Answer:
Hi..
Welcome to HEALTHCARE MAGIC..

I have gone through your query and can understand your concern..

As per your complain and presentation of symptoms it seems that the decay from your premolar tooth has deepened and reached the central portion of the tooth known as PULP which contains nerves and blood vessels..

The irritation of nerves leads to severe pain..

In your case it seems that the infection from the Pulp has extended to the area below the root tips via root canals and due to infection there is Pus formation.

This leads to formation of DENTAL ABSCESS and when the pus and infection puts pressure over the nerve which passes from the area of infection an lead to numbness of lip..

The pain in neck can be due to either Inflammation of lymph nodes due to tooth infection or radiating pain from tooth..

In this case painkillers does not work as the pus will continuously cause pressure over nerve endings causing pain..

You should immediately consult a Dentist and get evaluated along with an IOPAR (x-ray) to rule out the exact status of tooth and get treated accordingly..

In case if the tooth can be saved get the Root Canal Treatment of the tooth done followed by Antibiotics and Painkillers..
In case if it cannot be saved Extraction of the tooth can be done..

As of now you can start with antibiotics like Augmentin625mg thrice a day, painkiller like Tramadol 50 mg thrice a day and Pantoprazole 40mg once daily empty stomach.
Take for 5 days and only if there are no allergies..

But consultation with a Dentist is must..

You can also revert back to me with the x Ray report.

Wishing you soon recovery..

Any further questions are invited..

Thanks and regards..
