Brief Answer:
You should get an x-ray done to find out the exact cause..

Detailed Answer:
Hi..
Welcome to HEALTHCARE MAGIC..

I have gone through your query and can understand your concern...

As per your complain acute pain in tooth can occur due to a few reasons like:

1.In case if there is a deep decay in the tooth and the infection has reached the nerves which are located in the central portion of the tooth known as PULP CHAMBER..
The irritation and inflammation of the nerves leads to severe pain..

2.Other reason can be formation of a DENTAL ABSCESS in which due to progression of infection from the pulp via root canals to the portion of the tooth below the root tips.
The infection in that area causes pus formation and the pressure created over the nerve fibres by the pus can cause severe pain..

3.In case if the tooth is not decayed then pain in tooth that has occurred suddenly can be due to crack in tooth which can occur accidentally during chewing some very hard food or some trauma..
This is also a very painful condition..

You should consult an ENDODONTIST and get evaluated and he /she can also advise you to get an x-ray done to rule out the exact cause of pain..

In case if the pain is due to infection that has reached the nerves or if there is formation of dental abscess and if the tooth is in a condition that it can be saved then ROOT CANAL TREATMENT of the tooth followed by antibiotics [other than penicillin group as you are allergic to amoxicillin] and painkillers can help..

In case of cracked tooth if the crown portion is cracked then root canal treatment followed by capping or post and core treatment can be done..

In case of root fracture extraction has to be done..

As of now you can take TRAMADOL 50 mg thrice a day for pain relief..
But do not take for more than 5 days..

You should also revert me back with an x ray so that I can guide you better towards correct treatment..

You should also send me answers to a few questions like:

1.Is there a decay in the tooth or tooth appearing black?
2.Did the pain start after biting on any hard food product?
3.Is the tooth loose or moving somewhat?
4.Is there swelling on the offending side?
5.Which painkiller have you taken till now?

Waiting for your response.

Hope this information helps..

Thanks and regards..
