Suggest Treatment For Severe Muscle In Shoulders When Diagnosed With Fibromyalgia

Question: Hello. I suffer with fibromyalgia for 25 years. I have a lot of muscle pain all over but worst in my shoulders.
I am 71 years old. Can Alcohol effect muscles all over. i am taking 600mg of lyrica a day.
I have had Cortozone ?? injections into my shoulders on several occasions.No effect.
I have had MRI scans on my shoulders and nothing bad was found.
I had an operation on my back recently to repair a Vertebral Compression fracture using Baloon Kyphoplasty
However this is not causing me additional pain. It appears no one can help me. Regards, XXXX.

I have gone through your query and understand your concern.
Fibromyalgia is basically a disease which has both musculoskeletal as well as psychological problems and both of these should be taken care of while planning treatment for the same. Basically it has no definitive cure but if the symptoms are controlled, the patient feels better. Hence the treatment is directed more towards controlling the symptoms.
You are taking lyrica which is an approved drug for fibromyalgia. You should continue taking them. Alongwith, you have also mentioned taking many painkillers. You can take them after being prescribed by your treating doctor for pain control.
But apart from these, I would suggest duloxetine which is an antidepressant used effectively for fibromyalgia. You can consult this with your doctor and get it prescribed.
Also, I would suggest physiotherapy in form of shoulder, knee and hip range of motion exercises with spinal stretching strengthening exercises. Reduce intake of alcohol, caffeine and smoking. These might increase pain of fibromyalgia. Also relax; anxiety has been found to be associated with increased symptoms of fibromyalgia.
Get a blood sugar test done.

Hi there This is XXXX from XXXXXX
I am already taking Duloxetine 60 mg / day via a Psychiatrist with whom I am also taking Seroquel 150mg/ day and also Zyban 150mg x2 / day.
Physiotherapy. I have tried this several times and every time my pain has got worse. The Phyotherapist advised that I should stop. She reported back to my Consultant.
Please believe me there is almost nothing that has not been tried.
As the movie says " Is this as good as it gets ".

Sorry for the late reply.

Well if this is your condition, I believe you should continue with your medications.
Physiotherapy has worked well in almost all patients with fibromyalgia I have encountered before. In some, I had advised TENS or transcutaneous electrical nerve stimulation and it had been useful. You can consult your treating doctor about it.
For the rest, I should say that it wont be cured forever, The pain and symptoms are to be reduced and the treatment should aim towards that and a combination of drugs and physiotherapy is the preferred choice.
Also get the blood sugar test done. Sometimes, shoulder pain might be because of frozen shoulder seen commonly in diabetic patients.
