Question: dear sir... here i post this quiery for getting an idea about my brothers character and behaviour...

he is 25 ys old and married. he did his postgraduation from IIT and doing phd in london.. stayin wt wife at there..
he is facing high mood swings.. his energy level is high and he is highly committd with his work..

but at the same he easly geting irritated. short tempered....
some times we can see he is very down... lack of motivation... like mood off... bt in this situatiin .., he is trying to over come it by hearing music or walking alone...
he is very impulsive on food... he need to have food frequently a day..
he is a drinking person.. but not addictd... his wife told me tht some times he is using ghanja marijuana leaf .. bt it only occasionaly.. and easily available at there,england..

in good mood he playing footbal long time and like swiming , work out too... and highly dedicated on his study as a addiction..
he is not disclose he is loving his wife.. he told she is an individual., no one depend any one... like manner..
he is not ready to express love towords her...
she can understand his mood swings..thts why she is adjustin.. and their marge is love marriage.
but now a dys.. she is disappointed too about this deferent typs mood..means sometimes happy and at the same any issue arise he getting down and mood off easly...

about his history.. he is highly studious... and about our father..., he is a alcoholic and undergone deaddiction treatmnt two times... eventhogh he didnt complete deaddiction... nw we have enf problem from his means fathers side too...

he is not attachd with dad, he is an enemy towards him at some extend.. and he has only a attachmnt with mom only..

he is maintaining physical relation with wife... and its essential to him..

and his future idea about life is changing.. i can understand he is highly fluctuating about his future goals...

sir..

i need to knw... is this charctor shows only his personalty type? or need to give a detail psychiatric examnination... ?
or sir has he any symptom of mood disorder like bipolar.. ?
bt he never shown an full level mania symptoms in my experience and his wife too said the same..
they have limitation to see a dr at there and he is not willing.. bt he now he is easly annoyed..but he cant understand its may be a disorder...

sir pls go through my text and give an advice...

I read your query and understand your concerns.

There are two possibilities as per this description.

The first is mood disorder particularly cyclothymia. This is kind of bipolar disorder where the mood fluctuations are not extreme like bipolar disorder. Here the presence of irritability, mood swings, short temper, failure to show commitment in relationship while showing high dedication on other aspect of life, feeling low, despair all indicates that he gets similar depression and manic symptoms like bipolar individual but of low magnitude.


The next best possibility is bipolar II disorder where the irritability and depression and prominent components and mania never takes place.

In case these symptoms are there since 18 years of age or before the possibility of personality disorder should be considered.

In my opinion he need help rather than any advice and proper psychiatric evaluation should be done at earliest. His non willingness is definitely problem but it could not be a reason for not seeking psychiatrist assistance.

thanku for your good and valuable advce... and convyed tht with this wife too

sir... she has some doubts
if he is not willing to take treatmnt,, have chance to worse this condition
means is this change to a full blown mania or deep depressiin.. have any possibility.. and can we expecta natural and gradual remission..?

next sir. , if it is bipolar.. wht about its prognosis with treatmnt and without treatmnt... cz we are facing major problm about his unwillingnsa to take treatmnet... sir .. i think u can understand tht... and have any way to understand him easly about his mood swings
i am sure that we r asking toomuch.. but now present we r helpless.. mainly he is at there... and his wife told me that he have no emotional on physical contact sex.. only he us taking such as a presurable activity... so nw she is goin to plan higher studies and plan to shift another city alone... now she is afraid, is there any chance to him maintain any xtra marital relationship.. we knw no one cant guess this.. but she told once she experienced tht a long ago... so is this attitude is any symptom of bipolar.. just asking... ?

about father.. we knw that hus problem is not only alcohol addiction bt too drug induced psychosis.. when he drink., then only he is rude and have delusion.. and making problem.. so we afraid..., tht is hia condition have to chnce to worse.., he may go like his father.. we read some points in media about bipolar have some heriditory i chances...

at last.. i am going to conclud that he is not good in money managemnt... he spend money with out any planing and next struggle due to debit... his present situation means doing phd is highly stressful... sir.. thankuu..and sorry for long essay

Any untreated illness bound to worsen over a period of time and nobody can be considered exception. As per the scientific data available the early onset of treatment significantly favors positive outcome and it should be done.

Natural remission do occur in mood disorder for certain duration but the disease returns with more severity after some time. I think you should not wait for spontaneous remission.

If treated the prognosis will be better but if untreated there is enormous risk. About 10% of deaths in bipolar patients are caused by suicide. The another major complications include physical assault on life partner and others in bipolar patients.

Increased libido and dis inhibition is often present in individuals with bipolar disorder and the risk of extra marital relationship is more for bipolar individuals than the general population.

I agree with the expensive behavior attitude and heredity. Around 50 % of individuals with bipolar disorder have one or other family member with bipolar disorder.
