Question: My boyfriend is 51 years old, he is complaining of pain on the top of the foot that radiates to the shin, some visible swelling. He also says his heart has been racing. Yesterday he had the chills with no fever.

I understand your concern and would explain that it is important performing a careful differential diagnosis of different causes that may lead to this symptomatology:

-inflammation, such as arthritis of the foot joints could lead to such symptoms. A food X ray study coupled with a complete blood count and inflammation tests would be necessary.
The fact that the massage helps with the pain is an indicator of inflammation or a ligament or joint disorder (musculo skeletal pain).

- a deep vein thrombosis, which could also explain the visible swelling. A doppler ultrasound of the leg vessels would be necessary to exclude this possibility.

- a pinched nerve, such as tarsal tunnel syndrome. A nerve conduction test (electroneurography) would be needed to rule in/out this disorder.

I recommend consulting with his attending physician for a careful physical examination and the above mentioned tests.
