Compressed nerve roots, spinal cord seems to be free.

Detailed Answer:
Hello again and thanks for providing the MRI report and images.

There are present degerative changes of the vertebrae and intervertebral discs at 4-5-6-7 cervical levels.

The spinal cord manifests two areas of malatia mostly because of the past compression.

There is noted also laminectomy (decompression) at the levels 4-5-6.

The spinal cord is surrounded by the cerebrospinal fluid spaces and seems to be no longer compressed, so the laminectomy is working.

Some nerves exit zones are narrowed due to degerative changes, so, I guess you still should have some symptoms such pain and/or sensibity changes, etc.

If you experience moderate to severe symptoms related to narrowing of the nerve roots exits, (neurological examination is necessary to undergo better this) and if conservative treatment ( painkillers and physical therapy ) fail to improve your symptoms, anterior discectomy and fusion may be necessary.

Question1: I examined the images of the MRI too, so, together with the report, the situation is clear for me.
Since the painkillers are working, it is advisable to continue with the conservative treatment.

Question 2: Numbness in feet may be related to the myelomalacia, but also may be caused by peripheral nerves compression (Tarsal tunnel syndrome).
Getting evaluated clinically by your Doctor is necessary to understand better this issue.

Question 3: If the anterior discectomy and fusion will be considered, in my opinion, two levels (C3-4 and C4-5) should be considered since there is moderate to severe compression.
Of course, there are risks with such surgical procedures, but most of the cases result in very good outcomes.
