Question: What could this be for a 45 year old male on the abdomen.. This growth is painless and also has skin thickened beneath

Thanks for your query and the very clear picture.

Such a lump as seen in the picture, on the abdomen of a 45 years old male can be:
- Most probably a sebaceous cyst as the skin and the whitish cyst classical of sebum is seen inside. Since the cyst is on the abdominal wall, it shows the blackening part of the skin due to friction.

- It is less likely to be a lipoma since lipoma does not grow in this fashion, nor causes the skin changes as seen here as it is very slow growing.

I would advise to have this excised in-toto as there are skin changes and can go into stage of infection.

Does this need a biopsy?

Well, anything that is removed from the body needs to be sent for biopsy.
Biopsy means histo-pathological examination, hence should be done as is always done. Part of the procedure.
