Brief Answer:
Typhlitis, retro-cecal appendicitis, and so on.

Detailed Answer:
Hi.
Thanks for your query.

It is good to know that the OBGYN is clear after biopsy and ultrasonography has already been done. This also rule out to a certain extent the few other causes of pain in lower right abdomen.

With this in mind I can think of the following causes for a pain in right lower abdomen with low back discomfort, side of abdomen and around waistline:

Colitis of the cecum also called a typhlitis.
Retro-cecal Appendicitis ( retro meaning behind the cecum).
Ilio-psoas abscess. ( muscles at the back ).
Urinary tract infection is rare situations.

I would advise you the following:
Blood- CBC, sugar, urea, creatinine.
Urine - routine, microscopy, culture and sensitivity.
Stool examination.
Repeat ultrasonography .
CT scan to confirm or rule out specific cause.

A course of an antibiotic, metronidazole, probiotic and so on.
If there is a complete response, nothing more needs to be done.

If surgical cause is found, it is better to get operated.
I hope this answer helps you to get an early and proper diagnosis so that further treatment can be guided.

The appendix was removed years ago. What is your thinking in having sugar checked.

Get all relevant tests done- course of antibiotic and metronidazole

Detailed Answer:
Thanks for your feedback.

I suggest you get all relevant tests done to rule out other possibilities.
A course of antibiotic and metronidazole.

Appendectomy history noted.
I have already mentioned above about blood sugar. Sugar test needs to be done as a routine test along with other blood test to be safer side.
