Question: Hi,
My son has a stomach pain since 5 weeks. Few times it was accompanied by severe hives and vomiting.
Appendicis was rule out back than based on ultrasound and x-ray. Instead he was diagnosed with constipation. We are still waiting for allergy test, but meanwhile he got today a severe pain in right leg. He still has abdominal pain and he does not think that leg pain originates in stomach. He also got a some fewer of 38.5 C. We don't want to rush to emergency again, but something seems not right and I am afraid that it could be dangerous? Could you advise? Concerned mother

Brief Answer:
5 weeks - long time- Clinical evaluation and CT scan of the spine and abdom

Detailed Answer:
Hi.
I have read and understood the history about your 12 years old son about pain in abdomen for the last 5 weeks, a long time to suffer and remain undiagnosed and untreated.
Few times it was accompanied by severe hives and vomiting- Appendicitis ruled out by ultrasonography and X-ray -
Diagnosed as constipation -
Now has developed severe pain in right leg.
Also got some fever of 38.5 degree Celsius.

All these symptoms indicate that he needs proper clinical evaluation, physical examination and investigations under the guidance of a Pediatric Surgeon.
I would advise you the following;
Tests of blood, urine and stool.
CT scan of the abdomen and spine as there can be a problem related to the spine giving something like a psoas abscess which in turn can give pain radiating to the right leg.
It can be retrocecal Appendicitis not picked by any ultrasonography.

I hope this answer helps you to get a proper diagnosis, and get a proper further treatment. Please feel free to ask for more or if you need further clarifications or if you feel there is a gap of communication.
