Suggest Treatment For Red Itchy Hives Under Eyes

Question: My mother suddenly developed red-itchy hives under her eyes about 3 weeks back. These hives have not healed yet. In addition, occacionally her eyes get watery and red.
So far, we have consulted an opthalmologist, an ENT specialist, and a dermatologist as well. All doctors say that it seems like an allergic reaction, but noone has been able to identify what is causing it.
Can you help identify the possible reason, and suggest confirmatory tests for the same? Meanwhile, what can be done to give some relief to my mother?

Hello,

I went through your mother's history and would like to know more about her as follows:

a. She has developed hives for lasts 3 weeks. Did she use to apply anything into/over eyes before that? Did the complaints start in both the eyes simultaneously or one after another?

b. You have mentioned her eyes get watery and red. Is this related to hives or such complaints were present even before?

c. What are the medications that she has been used over last one month? What are the medications that she is using right now? (for eyes as well as for other conditions)

d. Does she have itching into eyes or around eyes?

e. Did she have such complaints anytime before?

f. Can you please upload image of her eyes?

g. You have mentioned she has allergies to dust. How was dust allergy diagnosed? What are the symptoms of dust allergies?

h. Does she have any nose symptoms - running nose, nose obstruction, sneezing, nose itching, etc? Does she instill any medication into nose (which might be reaching to eyes via a connection named "nasolacrimal duct")? Did she have any nose surgery recently?

i. Does she have any other skin condition? Does she have mostly dry skin or oily skin?

j. What was the reason for consulting ENT surgeon? What was the diagnosis put by the ophthalmologist and dermatologist?

k. Were any reports done? Can you please upload the reports?

l. If no cause of allergy is found, please read this very carefully: the skin around eyes is very thin (one of the thinnest in body). So any allergy to cosmetics/chemicals first manifests around eyes. It may be shampoo, nail polish, soap, face powder, practically anything which comes in contact with skin. So please try to remember whether she has any exposure to cosmetics/change in cosmetic product within last 3-4 months.

Please provide above details in follow-up. It will help me to know her condition in a better way.

At present, I would suggest you as follows:

1. For itching, I would suggest her to use an antihistamine like levocetirizine or cetirizine. It will only control itching, but will not treat the cause.

Thanks for the prompt reply. Here, I am replying to some of your questions. I'll share test reports and previous consultation details shortly.

Answer: One after another, within a day's time

Ans. No such complaint existed before.

>> d. Does she have itching into eyes or around eyes?
Ans. Around eyes. Lower eye lids and area between eyesocket and cheeks, to be specific. I'll upload a picture as you suggested.

>> i. Does she have any other skin condition? Does she have mostly dry skin or oily skin?
Ans. Her face has mixed, so-called T-type, skin. Oily forehead and nose, while rest of her facial skin is dry. Generally, her skin is quite dry, possibly owing to her thyroid condition.

>> j. What was the reason for consulting ENT surgeon? What was the diagnosis put by the ophthalmologist and dermatologist?
Ans. Othalmologist suggested ENT consultation to check if any sinus symptums were causing any irritation. However, the ENT specialist did not find any evidence of that. I'll share the consultation finding and test results about the same shortly.

As you have suggested, my mother already has levocetirizine prescribed to her.
