Suggest Treatment For Persistent UTI In An Infant

Question: this is case pertaining to my nephew. He is 1 year 2 months old. He has got duplex kidney on one side and a cyst in the urinary tract. He gets constant fever and had the cyst removed around a year ago. The cyst has reappeared. I am attaching his reports. Presently doctors have suggested to remove the duplex Kidney. I would specifically like to know the way ahead. Is his survival a question right now? What are the chances that removing the kidney gets him on tract, keeping in mind that he is just over a year old.

I have seen the reports attached by you and it looks like your nephew is having a very severe form of reflux nephropathy of the left kidney where the urine from the bladder is going back to the left kidney. It has scarred the kidney as well. In such a case it is recommended to undergo the suggested operation to prevent recurrent urinary tract infections.
Now to answer your questions
1. Survival is not an issue as the right kidney is functioning well. However the surgical and anesthetic risk is present just like any other operations
2. Removal of the refluxing kidney will not impair development as the right kidney will compensate.
3. There is risk that the right kidney will hypertrophy and cause focal segmental glomerulosclerosis later in life. However this can be prevented with regular checkups
