Question: What should i do about either a sperm granuloma or a spermatocele.

I have been to the doctor the felt it and said it was one or the other, and i would need an ultrasound to determine which it is, so i went and had the ultrasound and they said it is one or the other, so that was no help.

But i am tired of having a painful testicle

Brief Answer:
Please read detail answer bellow.

Detailed Answer:
Hello
Thanks for your query, based on the facts that you have posted it appears that you have undergone vasectomy 12 years back and have pain in Testicle and Ultrasound Scanning has been reported to be either Epididymal Cyst or Sperm Granuloma.

I shall be happy if you can post me few more details to help me in making comments on your problem.
1) Since how long you are having the pain in testicle?
2) Can you palpate hard mass or a rounded swelling in Scrotum?
3) Is the pain very severe or it is just a discomfort in testis?
4) Have you done color Doppler Scan of Testicle to rule out any vascularity in the mass suggesting active inflammation or any other lesion in testicle ?.
5) Is it possible to upload Scan report?
6) Have you ever being prescribed anti inflammatory drugs and if so what is the response?

Looking at the history of Vasectomy being carried out there is high possibility of sperm granuloma..

Taking anti inflammatory medicines along with injection of Steroid locally
(Intra-Lesion) may help to reduce the pain .

If there is no response the surgical excision of mass is the last resort to resolve your problem.

Hope I have answered your query ,please feel free to ask if you have more questions I shall be happy to help you
Thanks and Regards.
