Question: hi my nut sack hurts and lower back. and my leg started hurting a lot I don't get a Bonner for the last 5 days and It takes a long time to ejaculate

leg and back are always involved in the referred pain of the genitals.If there has been a trauma to the nut sack then seek a doctor in ER and let them assess the genitals for you. if it is not the trauma then no worries, This happens quite often.Testis arise from the back of the body and they descend after that so when ever there is pain or fatigue to them due to excessive use the pain gets referred to the back.
Now inability to urinate indicates an obstruction and it might need a catheterization. Also a urine routine exam is needed to be done to sort out any UTI or genitals infections like urethritis etc.Get a complete STD panel assessment if you had been active recently in coitus.
Now the delay in ejaculation is i am sure not an organic pathology at age of 21 as ED and performance issues are mostly innocuous and psychological at this point. You need to limit the use of genitals by either masturbation or coitus to no more than twice a week. If the problem still persisted, seek a sex therapist and they will fix this ejaculation troubles for you.
Get to ER anyhow , get a scrotal ultrasound to rule out any emergency like torsion or swelling or any collection, if once ruled out, all you would need would be some pain killers, scrotal support and a good rest and balanced diet to supplement the testicles demand.
