Question: I have a condition of Bilateral Varicocele where in the left testicle pains and only on releasing the stool or smoking a cigarette does the pain go away. Have got a scan of the srotum done and the condition is mild to moderate. What is the way forward in getting this treated?

Brief Answer:
unlikely to be due to Varicocoele

Detailed Answer:
Hi
The kind of pain that you have described is extremely unlikely to be due to the Varicocoele. The ache that can be due to Varicocoele is usually dull, mild, and often relieved by scrotal support.
What you have is more likely a neuropathic pain, and apart from scrotal support, you can try using medications like pregabalin 75mg once daily to twice daily and maybe a mild analgesic like tramadol twice daily.
Surgery for varicocoele in patients with pain very rarely causes resolution of the pain and can make it worse and we reserve it only when there is Infertility due to a grade III Varicocoele.
Hope I have been of help.
Regards.
