Brief Answer:
it can be due to TMJ, further evaluation needed..

Detailed Answer:
Hello XXXXXXX welcome to HCM,
I've gone through your query and understand your concern,

All these symptoms which you've described, could be due to Temporo-mandibular joint dysfunction or as known as TMJ..

Although the previous visit to the Neurologist and MRI was normal as per your description, you should get evaluated in details to look in to this further.. good to know that ENT specialist had already looked in to it, but it's more than that..

It's a complex disorder, and you should schedule an appointment with an Orthodontist and/ Maxillo-facial surgeon for thorough examination of the facial structures and investigations (like CT scan of the face and few others as per the clinical findings)..

Mainstay of treatment is conservative only (very rarely surgery, only if the conservative measures don't work out, so don't worry), you can take Tab PARACETAMOL 650mg three times daily after food for next 4-5 days for temporary relief..

Take steam inhalation three times daily for next 4-5 days, it'll provide some relief fro those symptoms..

Oral splints or mouth guards (Occlusal appliances) can be applied, and observe how you stay..

Avoid eating too hard/chewy foods..don't stress yourself..

After the confirmation of the diagnosis, the further planning of therapy with a combination of medication i.e. Amitriptyline along with physio therapy and relaxation therapy can be planned by the specialist accordingly..

Let me know how you stay in the due course

Kind Regards
