Suggest Treatment For Pain And Burning Sensation On Chest

Question: Sir, I have pain and feel like burn in my chest from last 1 year. I have shown to cardiologist and he done my ecg, x Ray and sonogrphy abdominals. Everything come normal. Now he is asking for endioscopy. I feel burn when eat spicy or empty stomach and pain specially when I wake up in morning and do some movements. Pls advise should I have to go for endioscopy. Pls suggest option.

Many patients come to me who have problems similar to what you are experiencing and most of them have normal Chest X ray and ultrasound scan abdomen.

Endoscopy is an investigation which will show features of acidity like acid reflux or gastritis should you have it. The doctor wants to know if there is any ulcer in your stomach or duodenum. They will also assess any esophagitis due to acid flowing backwards from stomach to esophagus. In these conditions, Chest X ray and ultrasound scan abdomen are normal.

Acidity and acid reflux refers to conditions were the acidic environment in the stomach persists. This may be due to

1. excess acid formation,
2. less acid utilization (due to fasting) leading to acid pooling and
3. backwards flow of the acid into the oesophagus (also known as gastrointestinal reflux disease.

This acid is neutralized by the intake of meals regularly. Complications due to uncontrolled acidity may include gastric ulcers.

You often experience symptoms like pain, heart burn. bloating and vomiting due to acidity.

Acidity is controlled by taking small regular meals, avoiding fasting and spicy meals. Medications like proton pump inhibitors are given to suppress gastric acid formation.

Also you should stop taking alcohol and smoking.

Whether acidity or ulcer is present can only be known if you get endoscopy done.

Sir, pls suggest food diet and medicine to control gas and acidity before I go for endioscopy which I dont want to do for now. Pls advise which food items I have to avoid in dal and other food. Can I eat nonveg.

Thanks for writing back with an update.

For slight symptoms you can take Ocid 20 which is omeprazole and helpful in mild gastritis. It is to be taken 30 minutes before food.

For severe gastritis please take PAN 40 (Pantoprazole) which is a better medicine than Ocid 20. PAN 40 must be taken on empty stomach or at least 30 minutes before food. You can take this medicine for a month and watch.

In meals please avoid fasting for more than 4 hours. Have small snacks through out the day.

Dal is a good source of protein and you must take it. Avoid spicy foods and take curds at lunch to reduce acidity. You can eat non veg but try to avoid gravy if spicy and hot.

Please try and get endoscopy done within two months.

What would be the cost of endioscopy. Should I avoid kala -white chana, potato and rajma . Any specific food items to be avoided. Any specific food items should be taken to reduce gas and acidity

Endoscopy might cost 1200 to 1500 Rupees in XXXXXXX There is no need to avoid kala -white chana, potato and rajma when cooked in less oil and spices and if taken in normal quantities.

Following foods should be reduced.
1. citrus fruits, such as oranges, grapefruit, lemons, limes, and pineapple
2. tomatoes and tomato-based foods
3. garlic and onions
4. coffee and tea
5. alcohol
6. carbonated beverages
7. foods deep fried in oil

Foods to eat include non-citrus fruits like apple, banana and watermelon. You must take curds or yoghurt everyday.
