Question: hi, I am 27 years old male. In march 2015, my muscle spasm near to nasolabial fold when I smile at left side face. I found that muscle twitch when I try to smile only. Gradually, in July 2015, I found some sensation in left side face. I cannot express in words but it's come and go and left side face felt like unbalanced from right side. And now, I had some minor pressure in left eye. I have done fraxel treatment in december 2014. All this symptoms comes and goes. I am afraid whether it's bell's palsy, stroke or MS or any other disease. Please consult with medication for same.

Brief Answer:
Tell a few details.

Detailed Answer:
Hi XXXX,
I am Dr.Ajay Panwar,a neurologist,here to answer your query.
If you would have presented to me as an outpatient with this medical history,I would have examined you to observe a few things-
1)Whether your both eye lids close well and symmetrically or not?Try to close your eyes with full effort and ask someone to see if both the eyes close equally or there is some asymmetry between the two?
2)I would have checked you for double vision. Please tell if this is the complaint with you in any gaze you see,or not?
3)Is your face deviated to one side or not?
4)You said that these symptoms come and go.How frequently? In how much time they improve?

Relapsing and remitting symptoms should not be due to bell's palsy.
Stroke also appear to be a less likely possibility.
MS can occur relapsing and remitting but evidence of neurological deficit should be objective.

Please follow up with details so that I can answer your query well.

Dear Ajaysir,

thank you for your quick response.

hereby I share my inputs:
1. my both eye lid close well and symmetrically too. Also identified eyes closed equally also with the help of friends.

2. no sir. No any double vision.

3. In the public or whenever I am stressed, face try to deviated left side(minor). Most of state, it's normal. Adding to that, I felt numbness or tingling sensation on that side randomly.

4. minor tingling sensation come and remain 3 to 4 hours and sometimes more hours in stress. I even donot know whether it is called tingling , numbness or any other thing. But felt unbalance of both the side.

minor pressure in left eye comes twice/thrice in a week and remains half and hour to 1 hour. sometimes its twitch also.

and finally muscle twitch near to smile line when I try to smile. It's normally remains for 1 hour to 2 hours and then goes.

Thank you sir for ur help.

Detailed Answer:
Hi XXXX,
Thanks for being in follow up.

I have watched the video provided by you.Though,in the first view,it appears that you are having an action(smiling) induced hemifacial spasm, but on close watch I was able to make out that you had contractions on opposite(right) side also and that is a strong point against hemifacial spasm.

This is definitely not Bell's palsy, stroke or MS.
Most possible reason appears to be anxiety.

I suggest you to start doing regular mental relaxation exercises like meditation/yoga.At the same time, please be on a trial of anti-anxiety medicine like clonazepam.I am sure you will show improvement.
PLEASE TAKE MEDICINES UNDER A PHYSICIAN'S GUIDANCE ONLY.

Hope that I have answered your query. If you have some further questions,I shall be glad to answer else please close the thread, rate it and write a review as your rating will be of help to me.
