Question: Awoke with lower back pain when moving wrong (twisting, bending). No obvious precipitating injury. Same result on firm or soft bed. Symptom improves over time and movement, but returns next morning.

Detailed Answer:
Hi there.

It could be muscle spasm of the muscles of your back which is causing the pain. There could be associated Osteoarthritis of the spine.

Avoid bending forward or lifting weights. Apply hot water bag to the painful regions. Take Diclofenac and Thicolchicoside combination twice a day for 5 days after meals as this is pain killer and muscle relaxant combination for relief.

Lie on your back and lift each leg with the knee extended for about 10 seconds and repeat the same with other leg after keeping the other leg back on the bed. Do this exercise thrice everyday about 50 times.

Avoid sleeping in uncomfortable positions. Sleep on a bed with good room for you to lie comfortably. Don't use a thick pillow.

If this does not help the pain subside in a week then get an Xray of your lumbosacral spine and do review with me.
