Question: I am 9 days late for my period. Urine and blood tests are negative for pregnancy. Experiencing low back pain, abdominal cramps, nausea, heartburn, diarrhea, and headache. Very fatigued.

Hello,

A normal menstrual cycle can be delayed by up to 10 to 12 days in the absence of pregnancy.

The risk of pregnancy is high if you have participated in sexual intercourse unprotected during day 11 to 19 of the cycle. Day 1 being the first day of bleeding of the cycle.

Urinary pregnancy tests are not always reliable.

Likewise it's important to rule out abortive bleeds which are associated with cramps and low backache.

For clarification it's advisable to consider ultrasound abdomen at the earliest.

Diarrhoea could indicate possibility of infection.

It's advisable to consider medications at the earliest in your case.

I generally prescribe oral medications of antibiotics like Ofloxacin plus Ornidazole and Pantoprazole plus Domperidone to my patients with such symptoms

I do not have any spotting or bleeding so I do not suspect a miscarriage or abortive bleed. What are the medications you listed supposed to treat? I was on the birth control pill long term and stopped taking it 4 months ago. I had 2 regular periods since then on a 40 day cycle. Is it possible I could have PCOS? I am also following a keto genic diet and have gained weight instead of losing weight. I also have frequent urination. My doctor prescribed Macrobid antibiotic for urinary tract even though my urine was clear of white blood cells, so I did not understand why I should take it. Do you think I should?

I also have thick clear egg white cervical mucus

The low back pain is more on the left side

Hello,

The prescribed medications are supposed to treat infection.

The possibility of infection is very high because of the description given by you.

Frequency of urination associated with flank pain can suggest either urinary tract infection or a genital tract infection.

In either cases, a delay in menstrual cycle should have a prompt search for pregnancy. Only then comes the PCOS and other possibilities. The other possibilities being kidney stones.

To have a thorough clarification it's advisable to get an ultrasound abdomen at the earliest.

Absence of WBC in urine does not always rule out infection.

If your doctor prescribed antibiotics, it's wise to finish their course.

Review with ultrasound abdomen to determine further course of action.
