Question: hello doctor, i have scratch on my dick outer skin, where is opening of dick , and skin become so tight ,it cant be able to open skin form inner shaft .thanks ur reply very helpfull

From your query it appears that you might have infection in your glans leading to balanitis and causing constrictive phimosis which in simple words means inability of the foreskin to retract.

In mild balanitis, gentle retraction of the foreskin daily and soaking in warm water to clean penis and foreskin usually treats the condition. Warm saline baths and drying the penis also reduces the inflammation.

In most cases topical treatment with ointments is recommended.
Fungal infections usually cause itching. Options for candidiasis (fungal) infection are
(i) If candidal infection is the suspected cause then clotrimazole cream 1% or miconazole cream 2% can be applied twice daily till symptoms subside.
(ii) Oral medication fluconazole 150 mg one tablet orally if symptoms are severe.
(iii) Topical ointment imidazole with 1% hydrocortisone if there is marked inflammation.

Bacterial infections usually cause pus and discharge. If bacterial infection is suspected then neosporin or clindamycin cream applied twice-daily until the infection has resolved.

For severe infections antibiotics must be taken as per complete course after consultation with your doctors.

You might be having balanitis of fungal origin if betadine is not giving you any relief. For a fungal infection, you can apply an ointment containing anti fungal and steroid combination.

Please use CANDID-B cream: clotrimazole 1 % w/w, beclometasone dipropionate 0.025 % w/w.

You must apply the cream gently over the skin in affected areas twice a day. Please apply after washing your genital organs with warm water and wiping it dry.

This ointment must be applied for a month and it will take a week to show any response.

Recovery is slow and every day during bath you can do gentle retraction of the foreskin daily and soaking in warm water to clean penis and foreskin.

If the foreskin is tight even after a month then please consult the urologist.

Applying cream CANDID B is important. Other medications you can take are as follows.

For severe pain you can take tablet diclofenac 50 mg twice a day after food for 5 days.

If severe infection is there then better to take a course of antibiotics Amoxicillin NOVAMOX 500 mg 3 times a day after food for 7 days. Make sure you are not allergic to Penicillin and always consult your doctor before starting antibiotics.

Please also take tablet PAN 40 before breakfast for 7 days to avoid gastritis due to pain killer and antibiotics.

hellodoctor, gud morning ,i want to ask u that candid-b cream is for cutts on skin . and wht about the tightness of skin , becoz the problem is that the when i push back the skin from shaft its very tight ,becoz of that tightness the skin cutts from round.the major problem is the tightnees of skin , if tightness problem solves cutts also recover . thanks plz suggests me gud medicine becouse on january 15,2015 is my marriage date , i have lot of tension , i dont want to go local doctor becoze its very shame full for me. thanks,,,,plz help me

1. Apply Candid D cream as instructed. It will heal the fungal infection and also the skin changes.
2. Take Novamox capsule to prevent any bacterial infection.
3. Slowly wash your penis head with warm water and soap and then you can slowly try to retract the foreskin with a lubrication jelly.
4. If there is mild pain and discomfort then you can use 2 percent lignocaine gel and the lubrication jelly and try to pull it slowly without causing any injury or cuts to foreskin.
5. If it still does not retract then please consult the urologist for surgical opinions.

Please do not worry for marriage. There is a lot of time you can expect it to be treated by then if you follow above measures. Please do not hesitate to consult urologist if the medications do not work.
