Suggest Treatment For Insulin Resistance

Question: Hi sir. My name is XXXX and my age is 34. Recently I when I had sugar test results was positive . When I consulted doctor next day after taking janumet table results was less compared to previous day. He suggested me to go for c peptide test result is normal. He told that I am insulin resistance. Is that permanent curable? In lipidprofile test my good cholesterol are 25. If I increase good cholesterol insulin resistance will cure?

Noted your concern.

Type 2 diabetes is due to insulin resistance.
On can reduces insulin resistance up to certain extent with the helps of-
- Diet: calories and fat restricted diabetic diet. One should consult near by dietitian who can plan diet considering Body Mass Index.
- Regular exercise: Usually we recommend 150 min per week of moderate degree physical exercise.
- Medicines like Metformin, DPP4 inhibitors, pioglitazone.

Depending upon blood sugar levels (Fasting and postprandial) and HbA1c level future treatment plan can be decided.
Usually insulin resistance is for life time how you modify your life style is more important.

Your good cholesterol HDL is 25 it is a part of insulin resistance it doesn't mean that if there is increase in HDL cholesterol, there will always be decrease in insulin resistance.

Regular exercise and statin (Cholesterol lowering medicines) may help in Increasing HDL cholesterol level.

So now it's meant, I have type 2 diabetes which is not curable I need to take the Medicine life long..?

also when I am checking Sugar level regularly with glucometer, some times fasting it showing 90,113 and 125 it is varying which is below 125. where as after meals it is showing 110,115 amd maximum of 120.

for me fasting only showing high sugar level if compare with after meal, why it is like that..?

fasting means no food from past 8hr right then why sugar level high in blood..?

Yes you may have Type 2 diabetes as you have already started on combination of 2 anti-daibetic medicines. (Though diagnosis is based on blood sugar levels and HbA1c level).

Fasting blood sugar is maintained by glucose production by liver (hepatic glucose out put).
Post-prandial blood sugar level maintained by calories intake and glucose utilization by body tissues like muscle (peripheral tissue).

Insulin resistance may vary at 2 different level hepatic level and peripheral tissue level which is responsible for variation in two blood sugar levels (fasting more than post-prandial.

My final question is do I need use medicine life long or since it is insulin resistance it may curable permanently. I read in one of the forum insulin resistance is curable please answer me

It will depend upon your commitment towards life style modification.

I have already mentioned measures to decrease insulin resistance.
whether it will completely cure or not will depends upon how you work on your lifestyle modification.
Most of patients require variable anti-diabetic medications but its doses and number depends upon your blood sugar level and HbA1c.

For your blood sugar levels mentioned by you earlier, I may suggest to check Fasting and post-prandial blood sugar levels in laboratory once in at least 2 months and HbA1c once in 3 to 4 month. Depending upon it appropriate changes in medications can be done.
