Dear Mr XXXX


Pain is not usually present in uncomplicated pile/hemorhoid. Presence of pain in pile suggest the possibility of thrombosis(clotthing of blood within the enlarged blood vessels of pile). The principle of management is

1. Treat the underlying constipation

2. Local treatment of pile

3. Decrease pain by analgesics and other measures

As there is no bleeding, the pile is likely to resolve on its own. Apart from what you have been following, I would also suggest the following

1. Ensure adequate intake of water: 2-3 liter a day

2. High fiber diet: 2 servings of vegetable salad and 2 servings of fruits especially citrus fruit will be helpful. Ensuring high fiber diet is very helpful in preventing bleeding from pile and also reducing the size

3. Hot seitz bath: To a tub of lukewarm water add 2 spoons of antiseptic sloution. Sit in it for 15-20 minutes each session twice a day. It will relax the perineal and perianal muscle and will help relieve pain.

4. Analgesic like Ibuprofen will be helpful to reduce pain
