Brief Answer:
Get the biventricular pacemaker at the earliest.

Detailed Answer:
Hi XXXXXXX
Thanks for asking.
I am Dr. Prakash HM and I will be answering your query.

Based on your query, my opinion is as follows:

1. He needs to add a third lead through a biventricular pacemaker to improve the ventricular function. As his ventricular function is reduced to 20-22%, having a biventricular pacemaker would improve his ventricular function, better.

2. Family and social support is essential and encouragement and good care-taking is necessary. During chronic disease, episodes of depression, discouragement are common. However, involvement during those periods, from around should be more.

3. Break from work and leisure and social activities could help. Priorities needs to be discussed with him. If necessary, counseling can be taken by both regarding managing him and yourself.

4. No further medications can be taken as they could complicate his cardiac and kidney problems. Controlling of all chronic diseases is necessary and medications needs to be watched for any adverse effects or nephrotoxicity.

5. Counseling, meditation, relaxation, involvement in social activities and caretaker and most importantly family support is essential.

Do discuss with him regarding the above. Try to go to Quebec to get biventricular pacemaker. Avoid any further load on the heart.

Hope it helps.
Any further queries, happy to help again.
