Brief Answer:
You need surgical management...

Detailed Answer:
Hi,

Fissure is a common condition that happens due to straining and hard stools. It is easily managed with conservative techniques using stool softener and warm sitz bath. However if they have transformed in to a fistula, it needs surgical line of treatment. I recommend you visit a surgeon nearby. He/she will perform a per rectal examination to asses the location and extent of fistula. Antibiotic regimen will be started. Surgery may be planned as and when feasible.

Until then you should try sitz bath. Take plenty of fluids. And add lots of fibre rich food in your diet.

Hope I am clear. Let me know if you need clarifications.

Regards
