Question: I have experienced eye twitching and pulsating temples and around ear, with a loud throb that wakes me up. Forehead pain and tingling. Also experiencing at times heart fluttering and slight chest pain.

Since when are you having this problem?
Are you having continuous pain and twitching?
Do you get this episode only after wake up or through out the day?
Do you feel numbness over the head or face?
Do you have neck and shoulder pain?
Do you have history of high blood pressure?If yes ,are you checking blood pressure periodically?
Do you have history of migraine?
Is chest pain radiating to arm or back?Do you have any gastric problem?
Do you have any other medical illness like diabetes,thyroid,heart problems or ear problems?
Have you attained menopause?

Thanks for your answer Dr. Nalaparaju. Answers to your questions are below.

Have had problem for several years now.

Twitching seems continual throughout the day however some days is slight and other days more severe. Mostly seems to be at night when I lay down. I have a lot of tingling of scalp. Slight hair loss.

No numbness on face or head. Have headache in forehead a lot, not every day.

I have no shoulder pain, neck pain only from osteoarthritis in my neck for years.

No high blood pressure, yes I check it periodically.

No migraine history although father had a lot of issues with those.

Chest pain NOT radiating to arm or back.

No diabetes, thyroid is ok.

No ear problems, however this pulsating heartbeat is real severe in my ear/above my ear at times, mostly at night when laying down. It wakes me up with temple pain, above ear pain, and throbbing/pulsating.

I have had full hysterectomy, no ovaries no fall tubes no uterus.

Have had a lot of heart fluttering when I lay down. When I take one aspirin daily it seems to help.

Big history of heart disease, MI, lots of relatives died from heart issues.

Have some appendix area pain, I went to ER for it in 2010 and they found nothing. To me, it feels like my appendix is inflamed, I can tell because its really low. I do have sometimes pain in gall bladder area, and I take lemon water to relieve it. I took XXXXXXX and YAZ years back, and feel it affected me. But no proof. From time to time have severe pain in my calf area. But never had myself checked for clots or anything. Took birth control pill for many years prior to hysterectomy.

Hi,

Apologies for the late response and thanks for providing more details.

You have reported symptoms spread across multiple body systems. Each of those may or may not be related. Without physical examination and investigations I find it difficult to related them together. Therefore in my opinion you need a good clinical examination followed by set of tests. I shall give you some guidance with which you may be able to find solutions.

1. Eye twitching most often occur due to unknown factor. In most of my patients I wasn't able to find a real cause to explain those twitches. Nevertheless with physical exercise, meditation and mind relaxation techniques many of them were able to get over those twitches. You should consider those methods.

2. Scalp tingling may be related to the same factors that caused eye twitches. We need to check your blood sugar, thyroid hormones(if you have not done in last one year), vitamin B12 levels to make sure other treatable causes are not missed.

3. Neck pain secondary to ostoarthritis should be reviewed with X-ray's. They may be related to temple pain.

4. Throbbing pain within the ear on lying down is not normal. I will suggest carotid artery doppler scan to make sure there is no circulation impairments.

5. Changes in heart rate in lying down posture is physiological. However since it seems to be prenounced a base line heart check is recommended.

6. It's extremely rare to find inflammed appendix in someone with low intensity abdominal pain. I will agree with your ER doctors here. Ultrasound scans besides blood tests will help us identify if there are any treatable cause.

In short, you require a battery of tests starting with blood hemogram, vitamin b12, baseline ECG/echocardiogram and carotid artery doppler. Your local physician will know about those tests. I recommend you visit the doctor and discuss about them.

Mean time I would suggest you Multivitamin medication Once daily after lunch.Some times acid reflux can cause discomfort in chest hence I would suggest you proton pump inhibitors for acid reflux like Omeprazole(Prilosec) 30 minutes before breakfast.This is over the counter medication.
Avoid using high pillow during sleep which increases neck pain.You can use hot bag fomentation for neck pain.
