Both your cholesterol and triglycerides are high. You would require medicines in addition to healthy diet and exercise to reduce them to normal levels and to reduce the risk of cardiovascular disease. It is not caused by your smoking but smoking increases the risk of cardiovascular diseases associated with high cholesterol and triglycerides.

Detailed Answer:
Welcome to HCM

There are subdivisions of cholesterol – LDL, HDL and so on. HDL is good cholesterol – higher, the better. LDL is bad cholesterol – lower, the better. In the absence of any other risk factor 100 is ideal for LDL-C. In the presence of other diseases (especially Diabetes and/or established heart disease), 70 should be the aim.

Keep your weight to normal / even a little less
Quit smoking, if you are a smoker
Do regular EXERCISE – say walking or jogging - 30 minutes a day / at least 5 days in a week. Isometric exercise (like bull worker) is bad.
Healthy FOOD is a must. Avoid saturated fats – coconut oil, palm oil, butter, ghee, full cream milk...Avoid junk foods in particular – French fries have palm oil / Pizza has cheese / XXXXXXX sweets are made in ghee.../ Change to Poly-un-saturated oil – like sun flower oil. / Include Mono-un-saturated oil – Olive oil.

Triglyceride also is High = 200 to 499 / Normal = less than 150 / Borderline = 150 to 199./ Very high = 500 or higher. A full 14-hour fasting is necessary for proper interpretation
Obesity / Diabetes / Thyroid insufficiency / Alcohol / over eating / some drugs like Beta-blockers / Diuretics are a few other causes. It is generally genetic [inherited /running in family]

Eat fewer calories / Limit natural sugar / Eat small, frequent meals and do not skip meals
Avoid late-night snacking / Add omega-3 fats in your diet / add fish, soy foods, canola oil, flax seeds / limit potatoes, yams, beans, corn and peas
Lose weight if you are overweight / Do regular physical activity – at least half an hour a day / 5 days in a week
__________________________________________________________

Statin is the first choice. .. atorvastatin is the most common one; starting from 10 mg, the dose is up to 80 mg, depending on the individual needs… / If triglyceride is level is still high in follow up, your doctor will add fibrates… Fenofibrate or Gemfibrozil….
The treating doctor will give appropriate prescription and follow you up
