Question: My grandsons have had diarrhea for about two weeks. The older one (22 months) has lost body fat and muscle. His abdomen is very bloated , and he has been clingy and whiny. He does not have a fever, but he did have one two weeks ago. The younger brother is teething, just got over an ear infection for which he was given amoxicillin. He also had a fever two weeks ago.
When I kept them two weeks ago, I ended up missing work for four days due to fever and a stomach virus that gave me diarrhea. I am still experiencing loose stools and excessive amounts of gas. Both boys have also had dry skin and a sandpaper rash. The younger boy(11months) has a vicious diaper rash.

Hi,
Welcome to the HCM.
I have gone through your question and understand your concerns but don't worry. The most likely causes of such episodes of long lasting diarrhoea in toddlers and late infancy are viral gastroenteritis with associated lactose intolerance or parasitic infestations (Giardiasis or amoebiasis). So you may start the following treatment for your grandsons.

1. Oral rehydration therapy including lamonade soups, buttermilk, ORS etc to keep them well hydrated.
2. Hold milk and other lactose containing products for next 15 days.
3. Start Syp. Ofloxacin-ornidazole for treating infections for next 5 days.
4. Start zinc supplement such as Syp ZnD or Zinconia for early recovery. Give for 14 days.
5. Start Racecadotril granules thrice daily for reducing water loss.
6. Give regular home made feeds to maintain nutrition.
7. Avoid sugars and sweetened juices.

For diaper rash, kindly send a picture so that I may suggest the most appropriate treatment.
