Brief Answer:
There may be possibility of bipolar disorder, provide symptom details

Detailed Answer:
Hello Mr. XXXX,

Thanks for writing to us.

Depression is of two types. One is uni-polar depression in which only episode of depression. The other is bipolar depression in which sometime episode of hypomania or mania occurs.Symptoms of hypomania or mania include excessive irritability, anger outburst, decrease need of sleep, spending more money than required, feeling of grandiosity, physical or verbal abusiveness.

When depression occurs in bipolar disorder, treatment should include one of the mood stabilizer medicine like lithium, lamotrigine or quetiapine. Only antidepressant medicines(like escitalopram) may precipitate episode of mania or hypomania.

You have mentioned that he is out of depression but stays subdued! It means he is not fully recovered.Mood swings and aggressiveness suggest features of bipolar disorder.

He has been given Prestique(desvenlafaxine),(Methmild)methylphenidate and Nexito(escitalopram). Are these medicines are still going on?

Please revert back to me with his brief detail history. Onset of problem, any stress, what were the symptoms, which symptoms improved, current symptoms,study-work detail and so on.

Waiting for your reply to help you further.

Regards,
Dr.Chintan Solanki.
